Submit a Claim - Claim Instructions

To qualify for a payment from the Net Settlement Fund, you must submit a timely and valid Claim Form. The Claim Form must be submitted online or, if mailed, postmarked no later than October 29, 2025.

You may submit the Claim Form::
  1. By mail to the Distribution Agent at the following address:

Please Note: If you are a bank, broker, or other third-party nominee filing on behalf of your clients, please file your claim on the Nominees page and not through this page.



How to File by Email or Mail:

Fill out, sign, and date the Claim Form. then mail the Claim Form to the following address:

Please keep copies of everything you submit, as your claim may be audited, and you could be asked to provide additional information to process your claim.


Remember, all Claim Forms must be submitted online or postmarked no later than October 29, 2025.
